Fort Morgan Humane Society Fun Run/Walk Waiver and Release of Liability Event: Fun Run/Walk Fundraiser Location: Riverside Nature Trails Date: November 1st, 2025 Waiver and Release In consideration of being permitted to participate in the Fort Morgan Humane Society Fun Run/Walk, I, the undersigned, understand, acknowledge, and agree to the following: Voluntary Participation I acknowledge that my participation in this event is voluntary and that I am physically fit to safely participate. Assumption of Risk I understand that participation in a trail run/walk involves inherent risks, including but not limited to: falls, contact with other participants, the effects of weather, traffic, animals, and trail conditions. I knowingly assume all such risks and take full responsibility for my participation. Release of Liability I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release and hold harmless the Fort Morgan Humane Society, its officers, directors, employees, volunteers, sponsors, and event organizers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in or traveling to/from this event. Medical Treatment I consent to receive medical treatment deemed advisable in the event of injury, accident, and/or illness during this event and release all parties listed above from any claim arising from such treatment. Photo/Media Release I grant permission to the Fort Morgan Humane Society to use photographs, video, or other media of me taken during this event for promotional or fundraising purposes without compensation. Pets If I bring a pet, I certify that my animal is current on vaccinations, is under my control at all times, and that I assume all responsibility for my pet’s behavior. I release the Fort Morgan Humane Society from any liability related to my animal. Acknowledgment of Understanding I have read this waiver and fully understand its terms.
